Patient Information:
Mustafokulova D.S., born in 1974.

Medical History:
The patient was urgently hospitalized on December 8, 2024, in the Hepatobiliary Surgery and Liver Transplantation Department No. 1 in critical condition with symptoms of obstructive jaundice and biliary sepsis.

Anamnesis:
Since August, the patient experienced symptoms of obstructive jaundice. Previously treated at a private clinic. On August 30, 2024, a biliary stent was placed, resulting in short-term improvement. However, symptoms of obstructive jaundice recurred before hospitalization, accompanied by clinical signs of cholangitis, including fever and chills. On December 8, 2024, the patient was urgently admitted to our hospital.

The patient underwent prompt examinations, and the following diagnosis was made:

Tumor of the head of the pancreas, complicated by obstructive jaundice, purulent cholangitis, and biliary sepsis.
The previously installed stent was removed. The patient was admitted to the intensive care unit, an emergency ERCP was performed, and a new biliary stent was placed. Symptoms of purulent cholangitis and obstructive jaundice were successfully resolved.

Radical surgery was recommended. On December 20, 2024, under the guidance of Professor Akbarov M.M., the following surgical procedures were performed:

Pancreatoduodenal resection,
Internal drainage of the Wirsung duct and placement of a pancreatogastroanastomosis per departmental methodology.
Gastroenteroanastomosis and hepaticoenterostomy.
Placement of a nasoenterobiliary tube.
Drainage of the abdominal cavity.
Histological Analysis:
Glandular tissue: ductal adenocarcinoma with ring-shaped structure.

Postoperative Condition:
The postoperative period was uneventful. The patient was discharged home in satisfactory condition under outpatient observation.

Recommendations:
Follow-up examination in 1 month, consultation with an oncologist.
